Patient Portal Information and Access

The portal is easy to locate at our website (grey oval button near top of page). Use of the portal includes requests for appointments (live or telehealth), medical questions, refills on medications, access to labs and radiology reports, and billing questions. With the addition of telehealth and the continually changing COVID-19 situation, portal use is critical since your provider (and/or office personnel) might be responding from the office, or on some occasions, from their home. We NEED all of our patients (or a family member) to set up an account today by creating a username and password. Please take a moment to do this right NOW. There is no requirement to complete any medical history at this time. This will be your primary way to communicate with the office for any needs during this time period.

If you have any difficulty setting up your portal account or have questions on using any of the features we are here to help walk you through this simple process from 8AM-4PM everyday (closed for lunch 12-12:30PM) with dedicated staff at 678-494-4450.
